The Admissions Coordinator will work very closely with the Health Care Transition Manager and be responsible for any and all administrative duties that are necessary to execute all admissions and keep our community within compliance. This role will also be directly responsible for helping to generate long term care stay referrals and admissions

 

Essential Responsibilities:

 

  • Meet long term care census goals
  • Fully execute the completion of WEL’s long term care application and assist families in expediting required documentation for all long term care referrals
  • Provide backup coverage to other WEL SNF’s to coordinate incoming admissions, as needed.

Qualifications

Related Experience:

  • 2-4 years administrative admissions experience in a hospital or skilled nursing environment; Strong knowledge of Medicare, Medicaid and other insurances 

 

Education or Degree Required:

  • High School Diploma or equivalent; Some College preferred

 

Licenses, Certifications, Registrations: 

  • Valid Driver’s License; Current Car insurance
