The Wellness Nurse is responsible for oversight, monitoring and assessment of independent residents to help them maintain their current level of care or assist them with the transition to a more appropriate living setting. The wellness nurse also provides a level of care which includes general first aid, post hospitalization follow-up, counseling on resources for physical and emotional health, and general wellness assessment. The Wellness Nurse will also perform other duties as required.

 

Essential Responsibilities:

  • Monitor and continually assess Independent Living residents to obtain information to develop appropriate interventions.
  • Assess residents upon admission or readmission to update medical records as required.
  • Update medical records as required by appropriately documenting changes in resident condition, incidents and follow up investigations, re-admissions, and any other relevant updates as required by our policies and procedures.
  • Collaborate with interdisciplinary team members to develop and implement individualized care plans as needed.
  • Effectively communicate with residents, families and staff regarding resident changes related to medical or behavioral concerns and changes in level of care.
  • Assess residents annually to ensure they are at the appropriate level of care.
  • Make rounds to seeing residents, making observations and taking appropriate interventions as needed.
  • Upon request of security, respond to resident emergencies, assess resident’s current condition and determine appropriate interventions based on policies and procedures.
  • Timely completion and submission of reportable incidents.
  • Ensure that residents are aware of the services available to them and encourage them to participate in life enrichment activities.
  • Follow standardized infection control practices to prevent the spread of infection.

Qualifications

Qualifications include:

  • Minimum 2 years nursing in a senior living environment preferred.

Education:

  • Graduate of an approved school of nursing.

Licensure/Certification:

  • Current nursing license, current CPR certification.
