The Admissions and Outreach Coordinator will work very closely with the Care Transition Manager and be responsible for any and all administrative duties that are necessary to execute all admissions and keep the 120 bed facility within compliance. This role will also be directly responsible for helping to generate long term care stay referrals and admissions, and provide on-site hospital liaison support to Care Transition Manager and in-house admissions coverage to other WEL SNF’s.
Admissions and Outreach Coordinator Essential Duties and Responsibilities include, but are not limited to:
Related Experience: 2-4 years administrative admissions experience in a hospital or skilled nursing environment; Strong knowledge of Medicare, Medicaid and other insurances
Education or Degree Required: High School Diploma or equivalent; Some College preferred
Licenses, Certifications, Registrations: Valid Driver’s License; Current Car insurance
